Caldwell County ESD 5 partners with Emergicon to manage EMS billing and medical records. To ensure a fast, secure, and streamlined experience, all medical records requests must follow the process outlined below.

How to Request Your Medical Records
Patients and authorized individuals may request copies of EMS patient care reports by submitting the Medical Records Request Form.

Only the current Medical Records Request Form is accepted.

The form must be completed in full to ensure timely processing.

Requests from Law Offices or Records Retrieval Companies
All requests from attorneys, legal offices, or records retrieval companies must be submitted through ChartSwap, as required by Emergicon. This ensures secure transmission and standardized processing.
